Who is the handover for, and which dates does it cover?

Feeding and daily routine

Record the arrangements for your own pet. This tool does not calculate feeding amounts or schedules.

Food type, measured portions, meal times and allowed treats. Use separate notes for each pet.
Health and contacts

Discuss medicines and specific needs in person. Leave the current treatment plan with the handover.

Include the country code if you will be abroad.
Use a contact near the place your pet will be staying. Check their availability before the handover.
Use confirmed information. An empty field means no information has been entered.
List the medicines. Doses and directions belong in the current treatment plan from your vet.
